Convert a month, day, and year into a date of the form YYYYMMDD

Purpose: Convert a month, day, and 4 digit year into a 4-byte integer date of the form YYYYMMDD.

Notes: This is the inverse function to breakup_date.

Results: The return value is a 4-byte integer.

Error Conditions: No error checking is done.
